Sunburst Farms Community Cooperative Holiday Hayrides WAIVER RELEASE/WAIVER – PLEASE READ: I hereby waive and discharge all claims, actions and causes of actions by me, my heirs, executors and administrator against the Sunburst Farms Homeowners Community Cooperative, and its officers, sponsors, volunteers, organizations and any others connected with this event, for any injury, harm, or death, damages, loss or inconvenience, sustained as a result of my participation in the Sunburst Farms Holiday Hayrides, or any activities associated therewith. I am aware of the inherent risks of riding on an open trailer, and I assume all responsibility for my own safety. I agree to supervise my children and guests at all times during this event. I also consent to and permit emergency medical treatment in the event of injury or illness. I also give full permission for the use of my name and photograph in connection with this event. We need a wavier for every rider. PARENTS: Please list the names of minor children and their ages.
